Member Agreement & Health / Safety Acknowledgment

Effective Date: The day a Member submits payment for their initial Relationship Readiness Evaluation. Or, creates a Matchmaking Collective profile.

This Member Agreement & Health / Safety Acknowledgement (“Agreement”) is entered into between Matchmaking Collective (“Company,” “we,” “us,” or “our”) and the individual (“Member,” “Client,” or “you”). By enrolling in services, participating in introductions, events, or any activities provided by Matchmaking Collective, you acknowledge that you have read, understood, and agree to the following terms.

1. Nature of Services

Matchmaking Collective is a relationship introduction and coaching service. We provide curated introductions, social opportunities, relationship education, and related services. We do not guarantee romantic outcomes, compatibility, engagement, marriage, or relationship success.

All services are provided for personal development and social introduction purposes only.

2. Eligibility & Accuracy of Information

You confirm that:

You are at least 28 years of age.

You are legally single (not married or in an undisclosed committed relationship).

All information you provide is truthful, accurate, and current.

You agree to promptly update Matchmaking Collective if any information changes.

Providing false or misleading information may result in immediate termination of services without refund.

3. Relationship Readiness Evaluation, Database & Membership

Completion of the Relationship Readiness Evaluation does not guarantee acceptance into any Matchmaking Collective membership program.

If you successfully pass the Relationship Readiness Evaluation, you may be automatically placed into the Matchmaking Collective database. Being placed in the database does not guarantee introductions, matches, or client membership.

Client Membership may or may not be offered at Matchmaking Collective’s sole discretion.

4. Membership & Participation

Membership grants access to matchmaking consideration, potential introductions, and/or events depending on your selected program.

Introductions are based on availability, compatibility factors, and professional discretion.

No specific number of matches or introductions is guaranteed unless explicitly stated in writing.

Matchmaking Collective reserves the right to decline or discontinue service to any individual for any reason deemed appropriate.

5. How Matches Are Identified & Selected

Matchmaking Collective identifies and selects potential matches using a combination of:

Information provided in your application and Relationship Readiness Evaluation

Stated preferences, values, lifestyle, and relationship goals

Professional judgment and human curation

Ongoing feedback from dates and interactions

You understand that matchmaking is a subjective and discretionary process. Compatibility is based on multiple factors and professional insight, not algorithms alone.

Matchmaking Collective does not guarantee that every introduction will result in chemistry, attraction, or continued connection.

6. How Potential New Members Find Matchmaking Collective

Potential new members may find Matchmaking Collective through a variety of channels, including but not limited to:

Google search or other online search engines

Word of mouth referrals

Personal or professional referrals from current or past members

Networking events, social mixers, or business-related events

Social media platforms (Facebook, Instagram, LinkedIn, TikTok, etc.)

Other marketing campaigns or partner sources

We may use these channels to reach prospective members and ensure a diverse and curated membership community.

7. Fees, Payments & Refunds

All fees are clearly outlined prior to enrollment.

Payments are non-refundable unless otherwise stated in writing.

Missed appointments, late arrivals, or cancellations outside the stated policy may be forfeited.

8. Confidentiality, Information Privacy & Personal Information

Matchmaking Collective respects your privacy and handles personal information with care.

a. Information Collected

Matchmaking Collective may collect, receive, compile, and maintain personal, private, and public information including:

Name, contact information, age, photos, demographic details

Physical appearance and personal characteristics

Lifestyle habits, interests, and preferences

Educational, employment, and military history

Dating, relationship, and marital history

Personal views and opinions (values, politics, religion)

Health-related information voluntarily provided (non-medical)

(“Personal Information”).

b. Use of Information

Personal Information may be used for:

Matchmaking consideration

Member screening and evaluation

Internal notes and coaching

Communication regarding services

c. Disclosure

Limited and relevant Personal Information may be shared with potential matches, members, contractors, or partners in connection with services.

Matchmaking Collective may compile information from third parties.

d. Data Security

Reasonable safeguards are used, but absolute security is not guaranteed.

e. Member Responsibility

You agree to keep confidential any information received about other members.

f. Release

You release Matchmaking Collective from claims arising from collection, use, or disclosure of Personal Information.

9. Media, Photography, Audio & Video Recording Consent

You may be photographed, audio recorded, or video recorded during participation.

You grant Matchmaking Collective permission to use your image, voice, and likeness for marketing, promotional, website, and social media purposes, unless otherwise discussed and agreed to in writing.

You waive any right to inspect or approve finished content and release all related claims.

10. Background Check Authorization

You authorize Matchmaking Collective to verify identity and conduct background screening when applicable.

Membership may be denied or terminated based on screening results at Matchmaking Collective’s discretion.

11. Data Retention & Opt-Out

Information is retained only as long as reasonably necessary.

You may request updates, corrections, or removal from active use.

You may opt out of marketing communications in writing.

12. Conduct & Community Standards

You agree to:

Treat others with respect

Refrain from harassment, manipulation, or abusive behavior

Communicate promptly and respectfully

Violations may result in termination without refund.

13. Intimacy & Sexual Conduct Recommendation

Members are strongly encouraged to refrain from sexual activity unless in a mutually acknowledged committed relationship.

Matchmaking Collective is not responsible for sexual interactions between members. You hold Matchmaking Collective harmless from related claims.

14. Appearance Change Notification

You must notify Matchmaking Collective of significant appearance changes including:

Weight changes

Hairstyle or hair color changes

Facial hair changes

Cosmetic procedures

Failure to disclose may result in suspension or termination.

15. Life Status Change Notification

You must notify Matchmaking Collective if:

You are too busy to meet matches

You need to pause membership

You are exclusively dating or in a relationship

You are engaged or married

You are traveling long-term

Core beliefs or lifestyle have changed

Failure to disclose may result in suspension or termination.

16. Assumption of Risk

Dating and meeting new people involves inherent risks. You voluntarily assume all risks.

17. Release of Liability

You release and hold harmless Matchmaking Collective from claims arising from introductions, dates, events, or communications.

18. Health & Communicable Illness Acknowledgment (Including COVID-19)

You acknowledge risk of exposure to communicable illness and assume all related risks.

Matchmaking Collective is not responsible for illness or medical expenses.

19. No Medical or Mental Health Advice

Services are educational and not medical or therapeutic.

20. Termination

Matchmaking Collective may terminate participation at any time, for any reason, with or without notice. All fees remain non-refundable.

21. Governing Law

This Agreement shall be governed by the laws of the State of Arizona.

22. Entire Agreement

This document constitutes the entire agreement and supersedes prior discussions.

23. Acceptance of Terms & Electronic Agreement

Uploading a valid state-issued photo ID and submitting payment constitutes your electronic acceptance of this Agreement and confirms that you agree to be legally bound by its terms.

No physical signature is required.

24. Agreement Modifications

Matchmaking Collective reserves the right to modify this Agreement at any time. Continued participation constitutes acceptance of updated terms.
